What Are Altars of Lilith in Diablo IV?
Altars of Lilith are special shrines in Diablo IV that you can interact with a single time. When you do, you'll gain some kind of passive bonus such as a permanent increase to your stats or the ability to carry more Murmuring Obols. This passive bonus applies to all of your characters in that realm.

A Note on Altars of Lilith in Strongholds
Almost all Altars of Lilith can be found in the overworld, but a handful are located within Strongholds. Typically, these Altars of Lilith cannot be immediately accessed right away -- you'll have to complete a quest within the Stronghold to reach them.
In most cases, the Stronghold will have some houses or buildings that are initially locked. You'll get an objective to defeat specific enemies or destroy certain objects to unlock the Stronghold. For example, one Stronghold has sorcerers creating a storm, and this causes ice to block the doors. Once you complete this initial objective, you'll be able to reach the Altars of Lilith inside the buildings.
